Fill Out Patient Information
Click on me if you want me in a mailing group, exp for
mailing letters or cards for the Holidays etc.

1. Click on Primary Card Holder How to add Insurance Information
Very Important to fill in Billing Type and Billing Form Note: the little clip boards are to copy same field from
the patient file.

Fill out all the Primary cardholders information, the
information could be the same as the patients or it could be a spouse,
child or guardian. 3.
If the information is the same as the Patient click on the
Copy Info from Main 4.
Relationship to the Patient needs to be filled out, who is
this person to the patient, if it is the patient just use Self. 5.
Id Number comes from the Patient’s Insurance Card. 6.
Copay Percent is left 0 for all cases accept cash which is
always 100 percent or if the insurance company gives out a percent Copay
like Medicare at 20% 7.
Click on the Insurance Co. drop down and type the first
letters for the spelling of your insurance company, then when you find it
click on it to insert it. 8.
If your Insurance Company is not in there or it needs editing
click the Edit button that is located directly on the right of that field.
Click add new insurance to add a new insurance company from that form. 9.
If there is a
deductible fill that out 10.
Put in a billing type, billing type is another way to
sort, may be a Cash Case, Insurance Case, Medi for all Medicare, NF for
no-fault, WC for workers comp etc.
If your type is not in there click add/edit type on the right of
that box. 11.
Select a billing form, Cash, HCFA, and NY doctors may have
a NYWC form. 12.
New Case is where you may have One Patient with a WC case
and NF Case. You would click on add
new case and click the case you want to work on. 13.
Click next to work on your Secondary Card Holder
info. It is basically the same as
the primary cardholder. .
How to add Secondary Insurance Information Just click next or Secondary Card Holder and
repeat the basic steps you did under Primary Card holder.
Injury Diagnosis Page
This is Where you type in the Default Diagnosis, Injury
Date and Description and how the Injury happened. This is where you put the patients default doctor and
place of service. This page is where you choose a default procedure
code. Remember these default codes
are not necessary but here to make you life just a little easier. 1st add a cpt below. 2nd click on the white box under CPT and an
arrow will appear. Choose the cpt code with billing fee you want and click
on it. If this is a No Fault case make sure you check the B box
Yes and put the state in the Place Box. Also if you need to add remarks that will follow the
patient in making a new visit or payment type in the Remarks yellow box.

Guarantor Page
Guarantor page is where the patients bills are going to
be sent to. Exp: a child may have a
mom who has the insurance and the copays are sent to the dad at another
address You should now be ready to add a new visit. Click on the Add New Visit Link. .

Adding a New Visit If you have chosen default cpt codes they will already
be ready for you to save as a white line with the codes and fee already in
place. You can any or all of the fields in that line. You don not have to put a To Date in if the date is the
same as the From You can change the doctor of the claim at bottom right. Or you can just change the doctor of the group You can put the claim on hold if you don’t want it to go
to the print ready queue yet. You have 5 options you can click on to save the visit. 1. Cash Payment-if patient gives you cash to full fill
total debt of visits 2. Cash Pay/Print if patient gives you cash to full fill
total debt of visits and you need a receipt printed. 3. Check or Credit Card-if patient with one of them a
screen will pop up to add the numbers and give you the option to print and
save. 4. Check No Payment if there is not a Patient Payment at
this time. 5. Check
Alternative Payment if patient is paying more or less amount owed
for this visit.
Check not a visit if you don’t want this claim to count
as a visit.
